Region 5 collects $2.00 per athlete in any USA Gymnastics sanctioned event in Illinois, Indiana, Kentucky, Michigan and Ohio. 

 

Head tax checks are to made out to Region 5 USA Gymnastics and mailed within 48 hours of the sanction ending to:
